We are trying to migrate the version 9 system to version 12 patch 19 . During the post-migration script execution UUMGTRTCRM01 process getting the below error.

Navigate To: Usage>Migration>Sequencing monitor.

After logging check on the above error getting the below error. Refer the below screen shot for the same.

The architecture of the system is multi-tier (Database server and application server are different). 

 Any inputs on the same will be highly appreciated.
