There is no Add button for this screen.  Click on one of the existing rows and press the Insert key on your keyboard.  The you can enter your value(s) and click Save.

No, you can't.  10 is the maximum.

10 is the maximum number of Segments, in GL Setup > Options.  Maria's screen shot is for Segment Codes, which I believe is unlimited - but certainly more than 10.
