Ohio Worker's Compensation Report

We are in the process of undergoing an Ohio Worker's Compensation Audit.  They have requested an excel based report that contains the following fields:

Gross Wages     Section 125 deductions     Difference between Gross & 125 deducts      Construction Limit     Exempt Wages     Reporting Wages

I was wondering if anyone already has this report set up maybe in Office Connector, or would be willing to help me out?
