what if you create your own screen, the standard could be depending on some logic that can be missed when copying. A header screen usually does not contain many fields. 

Did you put an activity code on the header screen or fields?

ON some of the fields there are activity codes. The header screen does not have an activity code

Did you put your screen name on the header screen field of the window.?
