The employer will need to create this form on their own as per company policy.

I can only provide the explanation of bonus provision

Each Company's needs will be different when it comes to the content of the form, which is why there can never be a standard template.  The content will depend greatly on the Company's bonus policy, ie. if it is a guaranteed or discretionary bonus, what % of salary is expected, etc.  It is best to design your own form to meet your organisation's specific needs.
