Hi Jessica,

This looks like a Change Request form from Project Management. There are three ways the company info can print.

1. The default is from the company information found in File, Company Settings, General.

2. You could have a logo image specified in Tools, Options on the Letterhead tab.

3. It could be built in as part of the design, in which case you would need either your consultant or someone in your office who has and knows how to use Crystal Reports to modify the information on the design.
