Hello AU2022, 

I've reached out to our Sage 50 US support team. They've informed me that this issue is not specific to release 2023. It is an issue which is the result of Windows functionality in certain environments.  This issue is cosmetic in nature, meaning that even though the items are “highlighted” only the selected item will actually be used on the transaction. As a simple workaround the mouse can be used rather than the keyboard to select the item, as the issue only occurs if the user uses the arrow keys on the keyboard to scroll the item list.

It most certainly doesn't only occur if you use the arrow keys. This is with just typing the part number we need. Then you can't tell which one is selected as you can see in the picture several items are highlighted!
