How do you enter to this function ? From the general menu or by jumping from another function ?

access this function from general menu.
This is a customized development created in my local machine, then i created patch and patch it to another testing machine. So the above incident happened after the patch integration in Testing Machine. as usual i created menu item after the integration, then the 'pen' sign appear.

This exact issue has happened to us as well(Version 9).  The only difference being that it is happening on the Commitments screen which is a standard screen.  I am using a full DEV badge with all right and view access.  I have also tried a suggestion i saw where someone said to go the the Window of the screen you are on and check to make sure the Save button under the Predefined Buttons tab was set to YES.  No luck there either.  Would be really helpful to see a resolution to this issue.
